Case Management

Our Case Managers provide individualized coordination of medical and rehabilitation needs based on findings and recommendations obtained through regulated health care professionals. Case Management then utilizes this information to implement care plans to best suit our unique client needs. Our Case Managers then oversee, coordinate and manage ongoing care of our clients. Our Case Managers have a deep understanding of the health care system, excellent communication skills, organization and team approach.
